Patrick Evivie, MD, FACP, RPVI

Dr. Evivie is a board certified internist and a Fellow in the nation’s largest specialty organization, The American College of Physicians. Patrick Evivie, MD, FACP, was born in Nigeria, is a graduate of University of Benin, Benin-city, Nigeria. He completed his residency in Internal Medicine at Howard University in Washington, DC. Dr. Evivie is also a Registered Physician in Vascular Interpretation (RVPI), certified by the American Registry for Diagnostic Medical Sonography (ARDMS.

Dr. Evivie is Clinical Assistant Professor of Medicine at the University of North Carolina Chapel Hill, where as a volunteer faculty member he has been involved in training future doctors on the art of medicine during their rotations at PrimeCare Medical Center. University of North Carolina Chapel Hill inducted him into the Eugene S.Mayer Community service Honor Society in 2001.

Dr. Evivie’s expertise has been utilized in a number of research studies as a participant and an investigator. He was also responsible for setting up the Diabetes Resource Center at PrimeCare Medical Center currently being run in collaboration with Presbyterian Hospital. The American Diabetes Association and the National committee for Quality Assurance recognized Dr. Evivie for delivery of Quality diabetic care. The America Heart Association, American Stroke Association and the National committee for Quality Assurance also recognized him, for delivery of quality cardiovascular and/or stroke care. Dr. Evivie was Secretary of the Charlotte Medical Society from 2002- 2004, and served as president from 2006-2007.

• He is a member of the Board managers at the McCrorey YMCA since 2004.
• He is a Board member of the Association of Nigerian Physicians in America.
• He is a Member of the professional section of the American Diabetes Association.
• He is a Member of the Medical specialty quality review committee at Carolina medical center-Mercy


Hans M Kwaku MD
MPH,FACP


Dr Kwaku is a board certified internist and a fellow of the American College of Physicians. He completed his medical training at the University of Ghana Medical School in Accra,Ghana. Dr Kwaku, then proceeded to the USA where he did his medical residency at Columbia University College of Physicians and Surgeons-Harlem Hospital Center, New York.

After his residency, Dr Kwaku moved to Washington DC were he worked as a hospitalist at one of the DC area hospitals. Around this time, he entered the School of Public Health and Health Sciences of The George Washington University where he obtained a Master's degree in Public Health with special interest in epidemiology.

He currently works as a general internist at PrimecCare Medical Center in Charlotte, North Carolina. Dr Kwaku has academic interests in clinical medicine with special focus on diabetes and research.

His hobbies include reading, listening to gospel music, home improvement and playing tennis. He lives in Charlotte with his wife, Dr Mala Freeman-Kwaku who is an obstetrician/gynecologist and their three children.



Kim Neely, PA-C, MSPAS



Ms. Neely is the center's certified physician's assistant, with over fifteen years of experience. She received her BS in the Division of Health Science-Physician Assistant Program from Touro College in New York City. Kim resides in Charlotte with her two daughters. She enjoys exercising and surfing the net in her spare time.
